Client: Direct Care Pediatrics – 1434 E 9400 S #100, Sandy, UT
Plan Name: Pulirr Pro: Direct Care Pediatrics – Post-construction Dusting
Service Frequency: One-time
Total Service Area: 1,335 sq. ft.

1. Scope of Services

Pulirr will perform a one-time post-construction dusting of Direct Care Pediatrics' 1,335 sq. ft. facility. This service includes:

• Detailing walls with a wet rag and/or vacuum
• Detailing baseboards
• Detailing counters
• Detailing furniture (chairs, tables, beds, sofas, etc.)
• Detailing front of cabinets (not inside)
• Detailing floors
• Removing debris

2. Price & Payment

• The total cost for this one-time service is $640.80, based on $0.48 per sq. ft.
• Payment is due in full at the time of booking.
• This service does not include a subscription or recurring charges.

3. Scheduling & Access

• The cleaning appointment will be scheduled in coordination with Direct Care Pediatrics.
• The client is responsible for providing access to the facility on the scheduled date.
• Rescheduling must be requested at least 24 hours in advance.

4. Cancellation & Refunds

• Since this is a one-time service, cancellations made at least 24 hours in advance will receive a full refund.
• Cancellations made less than 24 hours before the scheduled service may be subject to a $50 late cancellation fee to compensate for crew scheduling and prep.
  

5. Service Guarantee

• Pulirr guarantees high-quality cleaning and attention to detail.
• Any service concerns must be reported within 24 hours of service completion.

If a valid issue is reported, Pulirr will return to address it at no additional cost.

6. Liability & Special Care

• Pulirr will exercise caution around all furniture and fixtures.
• The client agrees to notify Pulirr of any delicate items, special handling instructions, or restricted access areas.
• Pulirr is fully insured, but not responsible for pre-existing damage or unsecured items.

7. Modifications

• Any changes to the scope of service (e.g., additional areas or tasks) must be agreed upon in writing and may result in a revised quote.
• This agreement is specific to the Sandy, UT location of Direct Care Pediatrics and is non-transferable.
